The short answer

In the United States, a medicated intrauterine insemination cycle commonly costs about $500 to $4,000 out of pocket, according to a 2025 American Society for Reproductive Medicine committee opinion. The answer to how much IUI costs depends on whether the cycle is natural or medicated, how much monitoring is needed, which medication is used and whether sperm must be purchased.

The insemination itself may be only a few hundred dollars. The full cycle can also include consultation, ultrasound, blood tests, medication, a trigger injection, sperm preparation and a pregnancy test. Ask for the all-in cycle estimate, not only the catheter procedure price.

These are broad U.S. estimates, not guaranteed fees. A natural cycle using partner sperm can be near the lower end. A highly monitored cycle with injectable medication and donor sperm can exceed the ASRM range.

How much does it cost to do IUI in different types of cycle?

Natural-cycle IUI

The clinic times insemination around spontaneous ovulation, using an at-home LH test, ultrasound or both. There is no ovarian-stimulation medication, so pharmacy and monitoring costs may be lower. Natural-cycle IUI is not appropriate for every diagnosis.

Oral-medication IUI

Letrozole or clomiphene may be used to encourage follicle development. Ultrasound monitoring helps the clinic assess response and reduce the chance of proceeding with too many mature follicles. A trigger injection may time ovulation. Oral-medication cycles are usually less expensive than injectable cycles.

Gonadotropin IUI

Injectable gonadotropins can cost much more and require closer monitoring. Developing multiple follicles increases the risk of twins or higher-order multiples. ASRM guidance for unexplained infertility generally favors oral ovarian stimulation with IUI before IVF and discourages routine gonadotropin-IUI because cost and multiple-gestation risk can rise without a proportionate live-birth benefit in that population.

IUI procedure cost versus total treatment cost

IUI places a prepared sperm sample through the cervix into the uterus around ovulation. The procedure itself generally takes only minutes, but it is the endpoint of a cycle with several potential charges.

Before paying, confirm whether the price includes:

Initial fertility testing is not normally repeated every cycle, but it belongs in the first-year budget. If donor sperm is used, shipping, cryostorage and the vial itself may cost more than the IUI procedure.

IUI cost without insurance

Without coverage, plan for $500–$4,000 for a medicated cycle, plus any initial workup or donor sperm. Ask whether the clinic has a self-pay package. Sometimes an insurer’s negotiated charge is higher than the clinic’s cash price, although paying cash may not count toward your deductible.

Request the clinic’s cancellation schedule. If no follicle develops, or if too many develop safely to proceed, you may still owe for medication and monitoring even though insemination does not occur.

IUI cost with insurance

Insurance is more likely to cover diagnostic testing than fertility treatment, but plans vary. Coverage may include office visits and ultrasound while excluding sperm washing, insemination or fertility medication. State mandates do not apply to every employer plan and may not include IUI.

Ask the plan:

  1. Is IUI a covered benefit for my diagnosis?
  2. Is prior authorization required?
  3. Must I complete a specific number of timed-intercourse cycles first?
  4. Are monitoring, sperm preparation and the insemination billed separately?
  5. Is medication covered under the pharmacy or medical benefit?
  6. Is donor sperm excluded?
  7. Does IUI use the same lifetime fertility maximum as IVF?

The IUI cost with insurance can range from ordinary copays to the full cash price. “Covered” does not mean free if you have an unmet deductible or coinsurance.

How much does IUI cost overseas, including India?

In India, published 2026 cash prices commonly put an IUI cycle around ₹8,000–₹20,000, although hospital estimates can reach ₹50,000 depending on medication, monitoring and location. One provider lists ₹10,000–₹20,000 including cycle monitoring, the procedure and basic medication. Donor sperm, advanced testing and injectable medication can be extra.

Traveling internationally for IUI usually makes less financial sense than traveling for a higher-cost procedure because IUI is time-sensitive, relatively low-cost and may require several monthly attempts. Add flights, accommodation, local monitoring and the cost of returning for another cycle. If you are already living in India, verify that the clinic is registered under the country’s ART framework and ask for the full cycle price rather than the insemination fee alone.

When is lower-cost IUI not good value?

IUI is less invasive and less costly than IVF, but it bypasses only the cervix. Sperm must still reach and fertilize an egg in a fallopian tube. ASRM notes that it is less useful with severe tubal disease, moderate-to-severe endometriosis or severe sperm abnormalities. Age and diagnosis also affect the chance of success.

A cheap treatment with a very low probability of working can delay more appropriate care. Conversely, moving straight to IVF may be unnecessary for some patients. The useful comparison is not simply $1,500 versus $20,000; it is the likely cost, time and burden of each reasonable path for your clinical situation.

How many cycles should you budget for?

Pregnancy is never guaranteed in one IUI. Some treatment plans use three or four oral-medication IUI cycles before reassessment; the appropriate number depends on age, diagnosis, response and priorities. Mayo Clinic notes that IUI may be repeated for three to six cycles in some situations, but that is not a universal rule.

Multiply the true all-in cycle price, not the advertised procedure fee. Three cycles at $1,500 each cost $4,500, while three monitored injectable cycles at $4,000 each cost $12,000 before initial testing or donor sperm.
